Practitioners’ ethical orientation and responses vary between practice settings. Yet, currently, the ethics for midwifery practice that is explicit in the literature and which provides the ideals of socialization into practice, is that of bio(medical)ethics. Traditional bioethics, developed because of World War II atrocities and increased scientific research, is based on moral philosophy, normative theory, abstract universal principles and objective problem solving, all of which focus on right and wrong ‘action’ for resolving dilemmas. They exclude context and relationship. Personal narratives (...) of mothers and midwives contest the appropriateness of these accepted values and systems for childbirth because they induce conflict between work-place/service provider ethics and personal/professional midwifery ethics. In contrast to the disembedded and disembodied approach of biomedical ethics, an ethically adequate response in midwifery practice resonates more with the ethics of intimates, such as feminist virtue ethics. This discussion examines the emergence of professional codes of ethics, influences that shape contemporary midwifery ethics, and the adequacy of codes to actualize values embedded in the midwifery ethics discourse. It considers the traditions of professional practice, the impact of institutionalization on health care, the application of a code of practice as a recent addition to those traditions, and the strengths and weaknesses of codes of ethics as models for ethical responses. The ethics of strangers (normative theory and abstract principles) are rejected as inadequate, and the ethics of intimates (feminist-relational ethics) are proposed as a more adequate guide for well-woman centred midwifery practice. Schmitt y énonce la nécessité pour l’Allemagne d’un « Etat total », équivalent allemand à ses yeux de « l’État totalitaire » de l’Italie fasciste. Cette prise de parole aura un effet décisif (...) : trente-cinq représentants de la finance et de l’industrie allemandes, auditeurs de la conférence, adresseront une pétition à Hindenburg, président du Reich. Ils l’inciteront à appeler le « chef du grand mouvement » au poste de chancelier du Reich. Ce « mouvement » est le déjà criminel NSDAP : le Parti national-socialiste ouvrier allemand, le parti « nazi ». Le 30 janvier 33, Hitler sera chancelier. En neuf semaines, la conférence de Schmitt aura eu pour effet d’abattre tous les obstacles à l’avènement du Troisième Reich. La formule de « l’Etat total » a donc su transmettre aux temps et aux langages une charge à l’énergie insoupçonnée, porteuse des plus grands crimes. Ainsi est révélée la responsabilité centrale de Schmitt dans la mise en place du nazisme. Ce juriste, qui étrangement apparaît pour beaucoup aujourd’hui comme un penseur politique de référence, n’a d’abord avancé qu’une seule carte d’un jeu mortifère. Mais c’était une carte maîtresse, elle était dangereusement efficace pour faire passer le pire dans l’Histoire. Il était intéressant de lire en parallèle les « Journaux » de Schmitt et de mesurer l’attention portée par le juriste aux événements, dans les semaines qui précèdent et suivent l’arrivée d’Hitler à la chancellerie du Reich. Ce qui frappe est cependant la banalité quotidienne qui transparaît dans ces notes. Si ce n’est, peut-être, qu’un mot va lui paraître acquérir un poids nouveau : « Jude », « juif ». Participants in clinical research sometimes view participation as therapy or exaggerate potential benefits, especially in phase I or phase II trials. We conducted this study to discover what methods might improve cancer patients’ understanding of early-phase clinical trials. We randomly assigned 130 cancer patients from three U.S. medical centers who were considering enrollment in a phase I or phase II cancer trial to receive either a multimedia intervention or a National Cancer Institute pamphlet explaining the trial and its purpose. Intervention (...) participants were 32 times more likely to believe that the trial’s purpose was to examine safety and 60 % less likely to believe they would experience long-term benefit or cure. There was no difference in enrollment decision. However, while patients’ understanding of the trial’s purpose improved and expectations of long-term benefit diminished, half the respondents still believed they would experience long-term benefit or cure from participation. Therefore, we conclude that multimedia interventions such as this one may help oncologists to explain the risks and benefits of early-phase cancer trials in a way that patients can more easily understand, helping them to make more informed decisions about participation. But further research into other factors that influence patients’ beliefs about the outcome of enrollment is needed, both to modify the interventions and to determine how malleable patient beliefs are. A common feature of our world seems to be that in all cases of causation, the cause and the effect are placed in time so that the cause precedes its effect temporally. Our normal understanding of causation assumes this feature to such a degree that we intuitively have great difficulty imagining things differently. The notion of backward causation, however, stands for the idea that the temporal order of cause and effect is a mere contingent feature (...) and that there may be cases where the cause is causally prior to its effect but where the temporal order of the cause and effect is reversed with respect to normal causation, i.e. there may be cases where the effect temporally, but not causally, precedes its cause. Over the last forty years, philosophers have argued back and forth about backward causation. It requires a certain structure of time for something as backward causation to be not only possible but also to take place in the real world. In case temporal becoming is an objective feature of the world in the sense that the future is unreal, or at least ontologically indeterminate, it is impossible to see how backward causation can arise. Th e same difficulty does not hold (...) with respect to forward causation. For even though it is assumed according to one dynamic view of time, the instant view or presentism, that merely present events exist—and past events therefore are no longer real or have become ontologically indeterminate—such a view can still maintain that past events once were there to cause present events. Future events, however, are still to come, and being indeterminate or nothing at all, they cannot cause any events in the present. In other words, causation backwards in time can occur only if we think of time as static; that is, no objective becoming exists, and the world consists of tenselessly occurring future events that exist in the same sense as past and present events. Pp. 371. $39.95 (cloth). As the editor of this volume, Robert P. George points out in his foreword that this hook is yet another manifestation of the renewed and growing interest in natural law theory. But why this recent increased interest in natural law theory? What purpose is this theory supposed to (...) serve, or in what way do its proponents see it as an improvement on existing theories? First of all, it must be noted, as the variety of essays in this volume makes abundantly clear, that natural law theory, as it is presently conceived, most certainly does not represent anything like a unified, single theory of tightly knit principles and conclusions. In the present volume one can easily see a half-dozen or more different approaches to natural law theory. So different in tone and spirit are some of the essays that one can easily wonder if they have enough in common even to classify them as " family resemblances." But if they all in their different ways do indeed evidence, as the editor claims, an increased awareness of and interest in natural law theory, the question immediately arises, why this increased interest? What problems do the advocates of these theories hope to solve by them or what questions do they think their theories will better address than the theories currently in place? First of all, one may make a division which places the theories, however divers~ among themselves, into two general groups. The first of these groups wishes natural law theory to function as a general approach to morality. For this group natural law does the work of a moral theory. In this group I would place the essays of Joseph Boyle, Robert George, Russell Bittinger, John Finnis, and Hadley Arkes (probably). The second group uses natural law theory as a way of determining the connection between moral law and positive law or legal theory. The essays of Jeffrey Stout, Neil MacCormick, Jeremy Waldron, Michael Moore, Lloyd Weinreb, Joseph Raz, and Ernest Weinrib fall into this second category. In all there are twelve essays in this volume. Space will not permit me to give a detailed analysis and critique of each. At most I can only indicate very briefly some of their more salient features. The book is divided into four parts, the first of which is titled "Natural Law, Practical Reasoning and Morality." In the very illuminating first essay of this section, "Natural Law and the Ethics of Traditions," Joseph Boyle undertakes to clarify the question of the relation of natural law theory to tradition. Certain approaches to moral theory currently popular BOOK REVIEWS 147 which are called virtues ethics, of which Alasdair Macintyre would he an example, claim that natural law theory is tradition-dependent. If Macintyre and other virtues ethicists are correct, then since natural law theory is culture-hound, and since the culture in which it was horn and flourished, medieval culture, has irretrievably retreated into the pages of history, then natural law theory is equally dead, of interest, if at all, only in an antiquarian way. Boyle carefully points out that there is more than one sense of being tradition-dependent. It is true that natural law theory is tradition-dependent in the sense that it did indeed come into existence in a particular historic epoch and that its language and thought do indeed express the period of its flourishing, i.e. the medieval period. But this, Boyle is careful to point out, does not mean that its validity and meaning were exhausted with the end of that culture. Rather the tradition is a living tradition so that its rich resources can continue to be exploited as that tradition is enlarged by its application to present day moral problems. Boyle's conclusion then is that in some senses natural law is indeed dependent on an ancient and medieval tradition, hut not so totally dependent that it has lost all validity for contemporary moral theory. How might a human communication system be bootstrapped in the absence of conventional language? We argue that motivated signs play an important role (i.e., signs that are linked to meaning by structural resemblance or by natural association). An experimental study is then reported in which participants try to communicate a range of pre-specified items to a partner using repeated non-linguistic vocalization, repeated gesture, or repeated non-linguistic vocalization plus gesture (but without using their existing language system). Gesture proved more effective (measured (...) by communication success) and more efficient (measured by the time taken to communicate) than non-linguistic vocalization across a range of item categories (emotion, object, and action). Combining gesture and vocalization did not improve performance beyond gesture alone. We experimentally demonstrate that gesture is a more effective means of bootstrapping a human communication system. We argue that gesture outperforms non-linguistic vocalization because it lends itself more naturally to the production of motivated signs. (shrink)

Initial responses to questionnaires used to assess participants' understanding of informed consent for malaria vaccine trials conducted in the United States and Mali were tallied. Total scores were analyzed by age, sex, literacy (if known), and location. Ninety-two percent (92%) of answers by United States participants and 85% of answers by Malian participants were correct. Questions more likely to be answered incorrectly in Mali related to risk, and to the type of vaccine. For adult participants, independent predictors of higher scores (...) were younger age and female sex in the United States, and male sex in Mali. Scores in the United States were higher than in Mali (P = 0.005). Despite this difference participants at both sites were well informed overall. Although interpretation must be qualified because questionnaires were not intended as research tools and were not standardized among sites, these results do not support concerns about systematic low understanding among research participants in developing versus developed countries. The semantic view on theories has been much in vogue over four decades as the successor of the syntactic view. In the present paper, I take issue with this approach by arguing that theories and models must be separated and that a theory should be considered to be a linguistic systems consisting of a vocabulary and a set of rules for the use of that vocabulary.

Interpretations of family carer empowerment in much nursing research, and in home‐care practice and policy, rarely attend explicitly to families’ choice or control about the nature, extent or length of their involvement, or control over the impact on their own health. In this article, structural empowerment is used as an analytic lens to examine home‐care nurses’ interactions with families in one Western Canadian region. Data were collected from 75 hrs of fieldwork in 59 interactions (18 nurses visiting 16 families) and (...) interviews with 12 nurses and 11 family carers. Generally, nurses prioritized client empowerment, and their practice with families appeared oriented to supporting their role and needs as carers (i.e. rather than as unique individuals beyond the caring role), and reinforcing the caring role through validation and recognition. Although families generally expressed appreciation for these interactions, a structural empowerment lens illustrates how the broad context of home care shapes the interpretation and practice of empowerment in ways that can, paradoxically, be disempowering for families. Opportunities to effectively support family choice and control when a client is being cared for at home are discussed. (shrink)

Probabilistic Relational Models (PRMs) are a framework for compactly representing uncertainties (actually probabilities). They result from the combination of Bayesian Networks (BNs), Object-Oriented languages, and relational models. They are specifically designed for their efficient construction, maintenance and exploitation for very large scale problems, where BNs are known to perform poorly. Actually, in large-scale problems, it is often the case that BNs result from the combination of patterns (small BN fragments) repeated many times. PRMs exploit this feature by defining these patterns (...) only once (the so-called PRM’s classes) and using them through multiple instances, as prescribed by the Object-Oriented paradigm. This design induces low construction and maintenance costs. In addition, by exploiting the classes’ structures, PRM’s state-of-the-art inference algorithm “Structured Variable Elimination” (SVE) significantly outperforms BN’s classical inference algorithms (e.g., Variable Elimination, VE; Local Conditioning, LC). SVE is actually an extension of VE that simply exploits classes to avoid redundant computations. In this article, we show that SVE can be enhanced using LC. Although LC is often thought as being outperformed by VE-like algorithms in BNs, we do think that it should play an important role for PRMs because its features are very well suited for best exploiting PRM classes. In this article, relying on Faÿ and Jaffray’s works, we show how LC can be used in conjunction with VE and deduce an extension of SVE that outperforms it for large-scale problems. Numerical experiments highlight the practical efficiency of our algorithm. (shrink)

Mais, à sa conception musicale encore partagée par Nietzsche, l'auteur oppose une conception visuelle de la pensée, qui prend sa source conceptuelle dans l'oeuvre de Giorgio de Chirico, inventeur de la peinture métaphysique. Dans un second temps, ce livre (...) esquisse une série de perspectives de recherches visant à déployer le concept de philosophie-artiste dans toutes ses dimensions : la primauté de "la narration philosophique", avec J.-P. Can reason play a significant role in making moral distinctions and in generating moral precepts? In this book I attempt to provide Hume's answers to these questions in the light of his employment of the 'Experimen tal Method', his doctrine of perceptions, and his analysis of reason. In addition to this, attention is paid to some of Hume's rationalist predeces sors - most notably, Samuel Clarke and William Wollaston - in order to assess Hume's critique of the rationalists. The aim of the work is to present the conservative views of the journalist and historian Henryk Lisicki and the role he played in the community with which he collaborated, i.e. the Krakow conservatives called the Stańczyk Circle. The source of the article are monographs, brochures, dissertations, essays and articles published in „Przegląd Polski“ by Lisicki. In order to evaluate to what extent Lisicki’s political thought was original, and to what extent it was in conformity with the Stańczyk Circle, the (...) publications of Walerian Kalinka, Stanisław Tarnowski, Michał Bobczyński, Józef Szujski and Stanisław Koźmian were also used. While working on the topic, the author made use of the works specializing in the history of Polish and Western conservative thought published by Ryszard Ludwikowski, Bogdan Szlachta, Jacek Bartyzel, Józef Szacki and the views expressed by the members of the Stańczyk Circle such as Michał Jaskólski, Wojciech Dutka, Stanisław Filipowicz, Henryk Michalak. When examining the above issues, a comparative method was used, tracing both the similarities and differences in the ideas of Lisicki and his colleagues from the Stańczyk Circle. The main thesis formulated in the work is that although Lisicki did not belong to the group of leaders, his close intellectual ties with Stanisław Koźmian gave him a respected position among Krakow conservatives. The analysis of his works confirmed that he was also capable of original reflections which focused on the attitude of the conservatives to the nineteenth-century democratization processes, the evolution of Western absolute monarchies towards the constitution democracies and the expansion of electoral rights. Hence, it was symptomatic for Lisicki and his community to argue, inter alia, for the necessity of rapprochement with moderate liberals, the aim of which was to weaken the danger coming from the revolted masses which were being under the influence of radicals. The patient preference predictor is a proposed computer-based algorithm that would predict the treatment preferences of decisionally incapacitated patients. Incorporation of a PPP into the decision-making process has the potential to improve implementation of the substituted judgement standard by providing more accurate predictions of patients’ treatment preferences than reliance on surrogates alone. Yet, critics argue that methods for making treatment decisions for incapacitated patients should be judged on a number of factors beyond simply providing them with the treatments they would (...) have chosen for themselves. These factors include the extent to which the decision-making process recognises patients’ freedom to choose and relies on evidence the patient themselves would take into account when making treatment decisions. These critics conclude that use of a PPP should be rejected on the grounds that it is inconsistent with these factors, especially as they relate to proper respect for patient autonomy. In this paper, we review and evaluate these criticisms. We argue that they do not provide reason to reject use of a PPP, thus supporting efforts to develop a full-scale PPP and to evaluate it in practice. There are no data in this work. (shrink)

A country‘s development is bound to be influenced by external occurrences. This article analyses two astronomical examples in which Portuguese nationals used high visibility events in the international scientific community to press their own scientific interests upon the government, whether these interests were, or were not, directly linked to the events themselves.During the 1840s and 1850s the parallax, i.e. the distance, of Groombridge’s star 1830 was hotly debated. The astronomer Hervé Faye‘s suggestion at the Académie des Sciences de Paris that (...) observations be made in Lisbon due to its privileged location and the inadequacy of the Lisbon Navy Observatory to perform them rekindled the efforts to improve the Portuguese Navy observatory.In the 1850s and 1860s other Portuguese higher education institutions also wanted to upgrade their facilities or build new ones. Following accepted traditions, scientific field trips were considered a useful part of the acquisition planning for larger instruments, and therefore an important part of regeneration and renewal projects. The Portuguese scientists were, however, limited in their travels by government imposed budget constraints. The total solar eclipse of 18 July 1860 provided an extra argument to send Portuguese scientists abroad. The government support of the eclipse expedition led to an opportunity to travel through other countries and view their observatories and equipment.In the end, neither the Navy astronomers nor those from the Coimbra Astronomical observatory, the other significant site of Portuguese astronomical research, managed to fulfil their objectives, but the contacts initiated by some of the Portuguese travellers were important for the development of the Portuguese national Meteorological and Magnetic Observatories. Since its founding in the nineteenth century, social anthropology has been seen as the study of exotic peoples in faraway places. But today more and more anthropologists are dedicating themselves not just to observing but to understanding and helping solve social problems wherever they occur--in international aid organizations, British TV studios, American hospitals, or racist enclaves in Eastern Europe, for example. In Exotic No More , an initiative of the Royal Anthropological Institute, some of today's most respected anthropologists demonstrate, in (...) clear, unpretentious prose, the tremendous contributions that anthropology can make to contemporary society. They cover issues ranging from fundamentalism to forced migration, child labor to crack dealing, human rights to hunger, ethnicity to environmentalism, intellectual property rights to international capitalisms. But Exotic No More is more than a litany of gloom and doom the essays also explore topics usually associated with leisure or "high" culture, including the media, visual arts, tourism, and music. Each author uses specific examples from their fieldwork to illustrate their discussions, and 62 photographs enliven the text. Throughout the book, the contributors highlight anthropology's commitment to taking people seriously on their own terms, paying close attention to what they are saying and doing, and trying to understand how they see the world and why. Sometimes this bottom-up perspective makes the strange familiar, but it can also make the familiar strange, exposing the cultural basis of seemingly "natural" behaviors and challenging us to rethink some of our most cherished ideas--about gender, "free" markets, "race," and "refugees," among many others.
